Swift Vein Institute -Henderson Will be Closing Effective December 31, 2024

Dear Valued Patient:

This letter is to inform you that we will be closing Swift Vein Institute, effective on December 31, 2024. While we remain available to provide care for you and/or your loved ones until December 31, 2024, it is important that you secure a new provider before that date to ensure uninterrupted care.  

For those patients who need continued care beyond December 31, 2024, telemedicine visits will be offered through Swift Vein Institute’s Reno Nevada office.

Please be assured that all medical records will be kept confidential and protected in accordance with federal patient privacy laws. With your authorization, a copy of your medical records can either be released to you or transferred to a physician of your choice. You have several options for obtaining these records:

• You may pick up your medical records at this practice by December 31, 2024
• You may request that your medical records via the “Request of Information” form found on the Swift Institute Patient Portal
• You may have your medical records sent directly to a physician or healthcare provider you designate by providing the name and practice address

To facilitate the release of medical records, you can contact Kristen Brehm by calling 775-437-1468 or emailing them at medicalrecords@theswiftinstitute.com. Please note that our staff will need 7-10 days to prepare your medical records and there may be a fee for record duplication.
