Physical Therapist, Remote
Michelle Forman, PT, is a licensed physical therapist who supports Swift Institute’s Remote Therapeutic Monitoring program from a remote setting. She focuses on neurologic, orthopedic, post-surgical, and cardiac rehabilitation, delivering care that is tailored to each patient’s individual needs.
Michelle earned her Bachelor of Science in Health Ecology from the University of Nevada, Reno, and completed her Doctor of Physical Therapy degree at Samuel Merritt University. Her final clinical rotation took place at Stanford Hospital, where she gained valuable experience in neuro and cardiac inpatient care. She holds certification in Neuro-Developmental Treatment (NDT) and has practiced in a variety of settings, including inpatient, rehabilitation, and outpatient environments.
Dedicated to helping patients restore function and confidence, Michelle takes a holistic, patient-centered approach to therapy. Outside of work, she is a mother of two and enjoys strength training, hiking, running, swimming, and celebrated completing her first marathon in 2023.
